Hepatitis B false-positive results: possible causes

Screening tests are designed to identify people who may need further testing. A reactive result may reflect current infection, a past immune response, recent vaccination, or a non-specific reaction. The meaning depends on which marker was detected, when the sample was taken, and whether other hepatitis B markers agree.

A clinician may repeat the test or order a panel of hepatitis B blood tests. They may also check liver enzymes with a liver function test and ask about symptoms such as dark urine or fatigue. One isolated result should not be used to label someone with chronic infection or to make decisions about close contacts.

Contact the clinician or laboratory that arranged the test and ask which marker was reactive. Provide the date of any recent hepatitis B vaccination, previous results, medicines, and relevant medical conditions. Until the result is clarified, avoid donating blood and do not share razors, toothbrushes, needles, or anything that may carry blood. Partners and household contacts can ask about testing and hepatitis B vaccination.

Arrange follow-up without delay if a screening result is reactive, even if you feel well, because hepatitis B can have no symptoms. Seek urgent help for yellow eyes, confusion, severe abdominal pain, repeated vomiting, unusual bleeding, or rapidly worsening illness. Those symptoms require assessment while confirmatory results are being arranged.

Ask which hepatitis B marker was positive, whether confirmatory testing or repeat sampling is needed, how vaccination affects interpretation, and whether your liver function test is reassuring. Ask when partners or household members should be tested or vaccinated.
