Hemorrhage and placental abruption: what to know

The placenta supplies oxygen and nutrients to the baby and normally remains attached until after birth. With an abruption, part or all of it separates early. Blood may leave through the vagina, collect behind the placenta, or occur with little visible bleeding. Pain, tightening, backache, or a firm and sensitive abdomen can be clues. The baby may show signs of distress because placental support is reduced.

Contact emergency maternity services immediately if abruption is suspected. Do not drive yourself if you feel faint or unwell. Lie on your side while waiting, keep your pregnancy records nearby, and tell the team about bleeding, pain, contractions, reduced movements, and any medicines. Assessment may include examination, monitoring of the baby and your vital signs, blood tests, and ultrasound. Treatment depends on bleeding, gestation, the baby’s condition, and whether birth is safest.

Seek emergency help for vaginal bleeding during pregnancy, sudden abdominal or back pain, repeated tightening, dizziness, fainting, a racing heartbeat, or reduced baby movements. Heavy bleeding is not required for this to be urgent: concealed bleeding can still be serious. Call emergency services if you are pale, weak, breathless, confused, or unable to reach maternity care safely.

Ask what findings support or exclude abruption, how the baby will be monitored, what warning signs should bring you back, and how delivery timing will be decided. If a caesarean birth is discussed, ask how it fits your situation and what recovery support is available.
