What should you know about height vertigo?

Short answer
Height vertigo is dizziness, unsteadiness, or a spinning sensation triggered by being high above the ground; it may reflect visual disorientation, fear of heights, or an inner-ear problem.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Looking down or standing high above the ground
- Useful distinction
- Spinning, faintness, imbalance, and panic can feel different
- Immediate safety step
- Move away from edges and sit or use a stable handrail
What is height vertigo?
Looking down from a balcony, staircase, bridge, or cliff can make the visual scene feel unstable. The brain compares information from the eyes, inner ears, and muscles and may receive signals that do not match. Some people feel pulled forward, light-headed, or unable to judge where the ground is, even when they are standing securely.
What it can mean
A brief episode only at a height can fit visual disorientation or an anxiety response. True vertigo can also occur at ground level and may be brought on by turning the head, rolling in bed, or changing position. Benign Paroxysmal Positional Vertigo is one possible inner-ear cause; Meniere's Disease may involve vertigo with ear pressure, ringing, or changing hearing. Migraine can also cause dizziness, sometimes without a strong headache. An ear infection may affect balance, particularly when ear pain or reduced hearing is present.
Notice whether the sensation is spinning, faintness, imbalance, or panic. Record what you were doing, how quickly it began, how long it lasted, and whether hearing changes, headache, nausea, weakness, or vision symptoms occurred. This pattern helps a clinician decide whether the symptom is mainly height-triggered or part of a broader balance problem.
What to do during an episode
Move away from an edge and sit or hold a stable handrail. Keep your gaze on a nearby fixed object rather than looking down, breathe slowly, and wait until your balance settles before walking. Do not climb, drive, or use machinery while dizzy. If episodes recur, arrange a medical assessment; treatment depends on the pattern and cause. A clinician may discuss balance exercises, migraine care, or the Epley Manoeuvre when positional vertigo is diagnosed.
When to seek care
Seek urgent medical help for dizziness with trouble speaking, facial drooping, new weakness or numbness, fainting, severe sudden headache, chest pain, or inability to walk safely. Prompt assessment is also sensible for new hearing loss, severe ear symptoms, repeated vomiting, or dizziness that persists after leaving the height. Arrange a routine appointment if the episodes keep returning, start happening on the ground, follow a head injury, or interfere with work, travel, or daily activities.
Questions for your doctor
Ask: Could this be a balance disorder or a visual response to height? What details should I record during an episode? Do my ear symptoms, headaches, medicines, or recent illness change the assessment? Is it safe for me to climb, fly, drive, or work at height while this is happening? Which examination, exercises, or treatment would be appropriate?

Questions people ask
Is height vertigo the same as fear of heights?
Not always. Fear may cause panic, trembling, and dizziness, while a balance disorder can cause spinning or unsteadiness even without fear. Some people experience both responses.
Should I see a doctor for dizziness only at heights?
Arrange an assessment if it is new, recurrent, worsening, or unsafe. Seek urgent help when dizziness occurs with neurological symptoms, fainting, severe headache, chest pain, or inability to walk safely.
Can vertigo happen when I am not at a height?
Yes. Inner-ear conditions, migraine, illness, medicines, and other causes can produce vertigo or imbalance in ordinary surroundings. The timing and triggers are useful details to share with a clinician.
