Heavy Brown Spotting: What It Can Mean and What to Do

Brown colour can appear when blood takes longer to pass and darkens as it is exposed to air. The amount, timing, duration, and accompanying symptoms matter more than colour alone.

Spotting may occur around the start or end of a period, after a change in hormonal contraception, or when ovulation changes the usual cycle. Irritation of the cervix after sex can also cause light bleeding.

Bleeding between periods can also be associated with infections such as Chlamydia, growths such as Endometrial Polyps or Uterine Fibroids, or changes in the cervix or womb lining. A cervical or womb condition can present with abnormal bleeding, but brown spotting by itself does not identify the cause.

If pregnancy is possible, bleeding needs individual advice because causes range from minor to urgent. Note whether you have pelvic pain, fever, unusual discharge, dizziness, or bleeding after sex.

Record when the spotting began, how often you change pads or liners, whether clots are present, and where it falls in your cycle. Also record contraception, medicines, possible pregnancy, pain, discharge, and recent sexual activity.

Arrange a clinician appointment if this is a new pattern, keeps returning, lasts beyond your expected period, follows sex repeatedly, or is accompanied by pain or discharge. Assessment may include a pregnancy test, examination, infection testing, and further tests guided by your history.

Do not use leftover antibiotics or vaginal products to treat unexplained bleeding. If you use a tampon and develop fever, vomiting, rash, or sudden illness, remove it and seek urgent medical advice.

Seek urgent care for bleeding that rapidly becomes heavy, faintness, severe or one-sided pelvic pain, shoulder-tip pain, marked weakness, or a positive pregnancy test with bleeding or pain. These symptoms can require prompt assessment.

Seek same-day advice for fever, worsening pelvic pain, foul-smelling discharge, or bleeding after menopause. Contact a clinician soon for bleeding after sex or repeated bleeding between periods, even when the amount is small.

Ask: Could my contraception, medicines, infection, or pregnancy explain this bleeding? Do I need testing for Chlamydia or another infection? Which changes should prompt urgent care? If the bleeding continues, what examination or imaging would help find the source?
