Heavy bleeding in early pregnancy at 4 weeks

Bleeding in very early pregnancy can occur for different reasons, including a pregnancy that is ending or bleeding from the cervix. A small amount does not always signal a serious problem, but heavy flow cannot be assessed safely from appearance alone. An ectopic pregnancy develops outside the uterus and can cause bleeding with pain, so it must be considered even when the pregnancy is very early.

Assessment may include a pregnancy test, blood tests and an ultrasound. At four weeks, an ultrasound may not yet show enough detail to answer every question, so repeat assessment can be needed.

Contact an urgent maternity or emergency service today for advice and assessment. Note when the bleeding began, how quickly pads become soaked, whether clots or tissue pass, and whether pain is present. Use pads rather than tampons so the amount is easier to observe, and avoid driving if you feel faint.

Seek emergency help immediately for severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, weakness, or bleeding that is rapidly soaking pads. These symptoms can indicate significant blood loss or an ectopic pregnancy and need urgent treatment.
