Heat rash on the scrotum: what to know

The scrotal skin is exposed to warmth, perspiration and rubbing from underwear or activity. When sweat ducts become blocked, a prickly rash may form. However, fungal infection, contact dermatitis, folliculitis and other problems can affect the groin and may need different treatment. Heat rash tends to be associated with sweating and may improve as the area cools; a diagnosis cannot be made from a description alone. Do not assume every itchy groin rash has the same cause.

Wash gently with lukewarm water, avoid scrubbing, and pat the area fully dry. Change out of sweaty underwear, choose breathable supportive clothing, and take breaks from activities that increase rubbing. Avoid perfumed powders, deodorants, harsh antiseptics and unprescribed combination creams on genital skin. Arrange prompt care for significant pain, swelling, sores, blisters, pus, fever, pain when passing urine, or discharge. Sudden severe testicular pain or swelling is an emergency and should be assessed immediately. See a clinician if the rash follows sexual contact, keeps returning, spreads to the groin, or does not improve after the area is kept cool and dry. Urgent help is also needed for breathing difficulty, facial swelling or a rapidly spreading allergic-looking rash.
