What should you know about heat rash on legs?

Heat rash on the legs is an irritation caused when sweat ducts become blocked, leading to itchy or prickly bumps and redness; cooling and keeping the skin dry usually help. It may appear after hot, humid weather, exercise, tight clothing or prolonged sweating. The rash can affect the thighs, behind the knees or areas where skin rubs together.

What heat rash on the legs can look and feel like

Miliaria, also called prickly heat or sweat rash, may look like small red or skin-coloured bumps, tiny blisters or patches of skin redness. Itching, stinging and a prickling sensation are common, especially when the skin becomes warm again. A heat rash is different from an infection, but scratching can break the skin and make infection more likely. Other causes of a leg rash include contact irritation, eczema, insect bites and some infections, so an unusual or persistent appearance deserves assessment.

What you can do at home

Move to a cool environment and use a cool, damp cloth on the affected skin. Take a lukewarm shower, pat the legs dry and change out of sweaty clothing. Choose loose, breathable fabrics and avoid tight trousers, heavy creams, fragranced products and activities that cause more heat or friction until the skin settles. Do not scratch or scrub the bumps. A bland moisturiser may help if the skin feels dry, while a pharmacist or clinician can advise whether another treatment is suitable for your symptoms.

Most mild cases improve after the skin is kept cool and dry. If the rash does not start settling, keeps returning, or makes sleep and movement difficult, arrange a dermatology review so the diagnosis and treatment can be checked.

When to seek medical care

Seek prompt medical advice if the rash is rapidly spreading, very painful, hot to touch, swollen or producing pus. A fever, feeling unusually unwell, red streaking, facial swelling or breathing difficulty needs urgent assessment. Contact a clinician if the bumps are accompanied by marked swelling, open sores, bruising, or a new medicine or product, because these features may point to another skin problem rather than simple heat rash.

Questions for your doctor

Ask whether the pattern and location fit heat rash, whether a cream could worsen blocked sweat ducts, and what signs would mean the rash needs review. Tell the clinician when it began, whether sweating or a new product preceded it, and whether there is pain, discharge or fever.

Questions people ask

Can heat rash appear only on the legs?

Yes. It can be limited to areas that become sweaty, covered, compressed or rubbed, including the thighs and behind the knees.

Should you moisturise heat rash on the legs?

If the surrounding skin is dry, a bland moisturiser may reduce irritation, but thick or fragranced products can trap heat and irritate the area.

When should a leg heat rash be checked?

Arrange medical advice if it persists, repeatedly returns, worsens, or is painful, infected-looking or associated with fever or feeling unwell.
