Heat rash on the knee cap

Heat rash occurs when sweat is trapped near the skin surface. Around the knee, kneeling, tight sports clothing, protective pads, and humid weather can add heat and rubbing. The area may feel itchy, prickly, or sore and can show tiny bumps, redness, or a slightly rough texture. A rash directly over the kneecap may instead be caused by friction, eczema, an insect bite, or inflammation around the joint, so persistent change needs a closer look.

Cool the skin, wash gently, and pat it dry. Pause kneeling and choose loose, breathable clothing while it settles. Wash and dry reusable knee pads, and place a clean soft layer between the pad and skin if equipment is necessary. Avoid scratching, harsh scrubs, and fragranced creams. A bland emollient can support irritated skin; a clinician can advise whether a topical corticosteroid is appropriate if redness and itch remain.

Have the rash assessed if it becomes hot, increasingly painful, swollen, draining, or difficult to bend the knee. Fever, rapidly spreading redness, blisters, or red streaks require prompt medical attention. If cooling and removing pressure do not help, the diagnosis may not be heat rash. A clinician can also check whether the joint itself is involved.

Ask whether the rash is limited to the skin, whether equipment or a product is contributing, and which cream is safe for the kneecap. Mention exercise, kneeling, new pads, insect exposure, and joint pain.
