Heat rash on the face after fever: what to know

A fever can make a person sweat, and damp skin, blankets, or head coverings may trap heat around the face. That pattern can lead to small prickly bumps. However, fever can also accompany viral illness, a medicine reaction, or another infection that produces a skin rash. The condition presents with heat rash when sweat and blocked ducts explain the bumps and the person is otherwise recovering; the presence of a fever does not prove the rash is heat rash. Note when the fever began, whether the rash followed a new medicine, and whether it blanches, blisters, spreads, or affects the mouth or eyes.

Keep the room comfortably cool, use light breathable clothing, and change damp bedding or headwear. Offer fluids as appropriate for the person's age and medical situation, and follow the advice already given for the fever. Clean the face gently and pat it dry. Avoid heavy ointments, fragranced products, and rubbing the rash. Do not start a facial topical corticosteroid or other medicated cream without advice, particularly if the cause is uncertain. Once the skin is no longer sweaty and the fever is settling, mild heat-rash irritation may ease; ongoing symptoms need a review.

Get urgent medical help for a rash with difficulty breathing, facial or tongue swelling, faintness, confusion, severe headache, stiff neck, marked drowsiness, or a rapidly spreading purple or bruise-like appearance. Prompt assessment is also needed for blisters, skin peeling, painful skin, eye or mouth involvement, repeated vomiting, or a person who looks very unwell. Contact a clinician if the fever persists or the rash does not improve as the illness settles. They can distinguish heat rash from another skin rash or infection.
