What heat rash on the calves means

Heat rash, also called prickly heat or sweat rash, can appear when sweat ducts become blocked and sweat is held beneath the surface of the skin. On the calves, warmth, humidity, exercise, tight clothing, boots, socks, or prolonged contact with a warm surface may contribute.

The area may look red or develop clusters of tiny raised spots, blisters, or bumps. It can sting, itch, or feel prickly, especially after further sweating. A calf rash is not automatically heat rash: irritation from clothing, contact dermatitis, folliculitis, insect bites, or another skin condition can look similar. Notice whether the change began after heat or sweating and whether it settles as the skin cools.

What you can do

Move to a cooler place and allow the calves to dry. Rinse gently with cool or lukewarm water, pat rather than rub, and wear loose, breathable clothing. Change out of damp socks or exercise clothes promptly. Avoid heavy oils, thick occlusive products, hot showers, and scratching while the skin is irritated.

A light, fragrance-free moisturiser may help if the skin feels dry or chafed; choose a product that does not leave a thick layer over sweaty skin. A pharmacist or clinician can advise whether a short course of a topical corticosteroid is suitable, particularly if inflammation or itching is prominent. Do not apply medicated creams to broken or infected-looking skin without advice.

When to seek care

Arrange a medical assessment if the rash is spreading, repeatedly returning, very painful, intensely swollen, or not starting to settle after cooling and avoiding the trigger. Seek prompt care if the skin becomes hot and increasingly tender, develops pus, crusting, red streaks, or you feel unwell, because these features can indicate infection or a different problem.

Get urgent help for sudden swelling of the lips or face, trouble breathing, faintness, or a rapidly spreading rash with severe illness. A clinician can examine the bumps and distinguish heat rash from other causes of calf redness or irritation.
