What it may mean

The buttocks are exposed to pressure, rubbing and limited airflow, especially during long periods of sitting or after sweating. Heat rash may appear as tiny bumps, redness or a prickling sensation. Irritation from fabric, detergent or personal-care products can look similar. Painful lumps, pus, a one-sided sore area, marked tenderness or a rash around the anal opening may need a different assessment; do not assume every rash in this area is heat rash.

What you can do

Wash gently with lukewarm water, pat dry and change out of damp clothing promptly. Wear loose, breathable underwear and take breaks from prolonged sitting when possible. Avoid fragranced wipes, harsh cleansers, scrubbing and picking. Keep skin folds dry, but do not apply powders to broken skin. A clinician or pharmacist can help choose a suitable soothing or anti-inflammatory product, particularly if the skin is cracked or close to mucous tissue.

When to seek care

Get medical advice if redness expands, the area becomes hot or very painful, pus develops, you feel unwell, or symptoms persist despite cooling and drying the skin. Seek prompt care for a rapidly enlarging tender lump, severe pain near the anus, fever, or difficulty passing stool. These features may indicate an abscess or infection rather than simple sweat irritation.
