Reading the skin changes
A patch beneath the pad may look red, dusky, mottled, dry, or bumpy. Heat and pressure can irritate the outer skin, and sweat can collect where clothing holds warmth. The pad fabric, rubber, adhesive, or a scented product may trigger contact dermatitis. If the rash extends beyond the contact area, is ring-shaped, or itches intensely at night, conditions such as ringworm or scabies need consideration. Psoriasis, atopic dermatitis, and intertrigo can also be aggravated by irritation.
The timing of the rash is useful: symptoms beginning during or soon after use suggest exposure, while a rash that continues to evolve after stopping needs assessment.
Protect the affected area
Stop using the pad on that site. Cool the skin with a clean damp cloth, cleanse gently, and pat it dry. Choose loose clothing and avoid scratching, friction, fragranced lotions, and direct sunlight while the area is sensitive. Do not place a heat pad over reduced sensation or damaged skin. If the rash is mild but not improving, a pharmacist or clinician can advise whether a soothing product is suitable.
Warning signs
Arrange a review when the rash persists after stopping the pad, spreads outside the treated area, recurs with ordinary warmth, or develops scaling, blisters, sores, or marked colour change. Seek prompt care for severe pain, skin peeling, pus, fever, facial swelling, or breathing symptoms. People with diabetes, poor circulation, or reduced sensation should seek advice earlier after a heat-related skin injury.
