The short answer

Heartburn is uncomfortable but is not the same as a heart problem; chest pain that is severe, unusual, or accompanied by breathing trouble still needs urgent attention.

What heartburn means in pregnancy

Pregnancy hormones can slow digestion and relax the lower oesophageal sphincter, the muscular valve that normally helps keep stomach contents below the oesophagus. As the uterus enlarges, pressure on the stomach can make reflux easier, particularly later in pregnancy. Heartburn can therefore occur even if you have never had reflux before.

Heartburn is a symptom of reflux rather than a diagnosis by itself. Persistent or troublesome symptoms may be assessed alongside conditions such as acid reflux, gastritis, hiatus hernia, or peptic ulcer. The pattern matters: tell your clinician how often it happens, whether food triggers it, and whether swallowing is painful or difficult.

Ways to ease symptoms

Try smaller meals more often, eat slowly, and remain upright after eating. Avoid lying down soon after a meal, and notice whether spicy, fatty, acidic, or caffeinated foods make your symptoms worse. Raising the head of your sleeping surface may help night-time reflux; use a safe arrangement that does not create a slipping or falling hazard.

Avoid tight clothing around the abdomen and ask a healthcare professional or pharmacist before taking an antacid, alginate, or other acid reflux medication in pregnancy. Some products are unsuitable in pregnancy, and medicines may need to be separated from iron or other supplements. Do not stop a prescribed medicine without discussing it with the prescriber.

When to seek medical care

Seek urgent help for chest pressure or pain with shortness of breath, fainting, sweating, marked weakness, or pain spreading to the arm, jaw, back, or shoulder. Urgent assessment is also needed for vomiting blood, black stools, severe abdominal pain, or repeated vomiting that prevents you keeping fluids down.

Contact your maternity team or clinician promptly if heartburn is persistent, is worsening despite simple measures, causes painful or difficult swallowing, or is associated with unintended weight loss. New severe upper abdominal pain with a severe headache, vision changes, or sudden swelling also warrants urgent pregnancy assessment rather than self-treatment as heartburn.
