Heartburn in the second trimester: what to know

Short answer
Heartburn in the second trimester can happen as pregnancy hormones relax the valve above the stomach and the growing uterus changes pressure in the abdomen.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the sensation means
Stomach contents can move upward into the oesophagus, causing a burning feeling behind the breastbone, a sour taste, burping, or irritation after meals. Pregnancy-related heartburn does not by itself show that the baby is unwell. Symptoms may be more noticeable after a large meal, when bending, or when lying down.
Persistent symptoms can also occur with conditions such as [Acid Reflux](/conditions/acid-reflux/) or [Gastritis](/conditions/gastritis/). A clinician can distinguish these from other causes by asking about timing, pain, vomiting, and medicines.
Ways to ease it
Try smaller meals, eat slowly, and remain upright after eating. Notice whether rich, spicy, acidic, or caffeinated foods trigger symptoms, then reduce those triggers if helpful. Avoid eating close to bedtime and loosen clothing that presses on the upper abdomen. Ask your maternity clinician before starting an antacid or other [Acid Reflux Medication](/treatments/medical-management/acid-reflux-medication/), because pregnancy-safe choices and timing depend on your health and other medicines.
When to seek care
Contact your maternity team promptly for severe or persistent upper abdominal pain, repeated vomiting, trouble swallowing, vomiting blood, black stools, faintness, or chest pain with breathlessness or sweating. Seek urgent help for sudden severe chest pain or difficulty breathing. Arrange a routine review if symptoms keep disturbing sleep, eating, or hydration despite practical changes.
Questions for your doctor
Ask which over-the-counter products are suitable in pregnancy, whether your symptoms could reflect another condition such as [Hiatus Hernia](/conditions/hiatus-hernia/), and how to separate heartburn from pregnancy warning symptoms. Bring a list of medicines, supplements, and foods that seem to trigger episodes.

Questions people ask
Can second-trimester heartburn harm the baby?
Heartburn itself is usually a digestive symptom and does not indicate that the baby is being harmed, but severe vomiting or dehydration needs medical attention.
Should I take a heartburn medicine during pregnancy?
Ask your maternity clinician or pharmacist to choose a suitable product and explain how to time it with other medicines.
