Heartburn in early labour: what should you know?

Heartburn in early labour is usually a digestive symptom, not a reliable sign that labour has started. Pregnancy-related changes, a full stomach, lying down, or pressure from the growing uterus can allow stomach contents to move upward. Labour is assessed through contractions, changes in the cervix, and other maternity symptoms rather than heartburn alone.

A burning feeling behind the breastbone, sour taste, or fluid coming back into the throat fits heartburn. It can occur before, during, or after labour without showing how labour is progressing. A pregnancy symptom can also be unrelated to contractions. Acid reflux is one possible explanation; gastritis or a hiatus hernia can also cause upper abdominal or chest burning. A clinician can distinguish these from labour-related discomfort.

Tell your maternity team about the symptom, especially if you are already having regular contractions or your waters may have broken. Sit upright, loosen tight clothing, take small sips of water, and choose a light meal if you can tolerate food. Avoid lying flat immediately after eating. Do not start an antacid or acid reflux medication without checking that it is suitable in pregnancy and compatible with your other medicines.

Contact your maternity unit urgently for heavy vaginal bleeding, reduced fetal movement, severe or persistent abdominal pain, suspected ruptured membranes, or contractions that become regular and painful. Seek emergency help for chest pressure, trouble breathing, fainting, vomiting blood, or black stools. These warning signs need assessment even when heartburn seems to be the main complaint.

Ask whether your pattern of contractions suggests early labour, which heartburn remedies are appropriate at your stage of pregnancy, and whether another cause such as gastritis needs checking. Mention when the burning began, what you ate, whether it worsens when lying down, and any medicines or supplements you use.
