Heartburn and ear pain: what to know

Short answer
Heartburn can irritate the throat and sometimes refer discomfort toward the ear, but ear pain needs its own assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Important distinction
- Ear pain may be separate from reflux-related throat irritation.
- Avoid
- Do not insert objects or drops into a painful ear without advice.
What should you know about heartburn and ear pain?
Heartburn can irritate the throat and sometimes refer discomfort toward the ear, but ear pain needs its own assessment. Reflux-related irritation is more likely when burning, sour fluid, hoarseness, or throat clearing occur together. Ear pain may instead come from an ear infection, wax, jaw-joint strain, dental trouble, or another throat problem.
Possible explanations
Acid reflux, gastritis, a peptic ulcer, a hiatus hernia, and factors that increase abdominal pressure such as obesity can be associated with heartburn. They do not automatically explain ear pain. Ear discomfort that worsens when chewing may suggest the jaw or teeth; reduced hearing or discharge points more toward the ear. A clinician can examine the ear, mouth, throat, and neck rather than treating the symptoms as one diagnosis.
What you can do
For mild burning, eat smaller portions, remain upright after meals, and note triggers such as late eating, spicy food, or alcohol. Do not put cotton buds, oil, or unprescribed drops into a painful ear. A warm cloth may soothe the outer ear, but avoid heat if there is discharge or a damaged eardrum is possible. Arrange an appointment if pain persists, recurs, or affects hearing.
When to seek care
Seek prompt care for severe ear pain, fever, swelling behind the ear, new hearing loss, dizziness, facial weakness, pus or blood from the ear, or trouble swallowing. Emergency help is needed for breathing difficulty, rapidly increasing neck or throat swelling, or severe chest pressure that could be mistaken for heartburn. Children and people with weakened immunity should be assessed sooner.
Questions for your doctor
Ask whether the ear, jaw, teeth, or throat appears to be the pain source; whether reflux is contributing; what treatment is safe for both symptoms; and whether an ear examination or hearing test is needed.
