Nicotine and heart palpitations

Palpitations may follow cigarettes, vaping, nicotine pouches or replacement products. Nicotine is not the only possible explanation: stress, caffeine, dehydration, illness and some medicines can create a similar sensation. Timing matters; note whether symptoms begin soon after use and settle as the effect wears off.

Recurrent symptoms still need a wider assessment. A clinician may check your pulse and blood pressure, perform an ECG and consider tests for Anaemia, Hyperthyroidism or Hypoglycaemia. A rhythm problem such as Atrial Fibrillation can occur with or without nicotine exposure.

Stop nicotine while the sensation is active, sit down and breathe slowly. Do not drive if you feel faint. Keep a record of the product, amount, timing and symptoms, including caffeine or decongestants used that day. If you want to stop nicotine, ask a clinician or pharmacist to help choose a suitable plan, especially if you are pregnant or have heart disease.

Call emergency services for palpitations with chest pressure, severe breathing difficulty, fainting, collapse or marked weakness. Seek prompt medical advice when episodes keep returning, continue after stopping nicotine, feel irregular, or occur with dizziness. New breathlessness or swelling also warrants assessment, including for Heart Failure.

Ask whether your nicotine product or other medicines could be contributing, whether an ECG or monitor is appropriate, and which nicotine-stop options fit your health history.
