Why you may notice them at night
Quiet surroundings can make a heartbeat easier to notice, and a dream, stress response, fever, dehydration, alcohol, caffeine, or low blood sugar may trigger an episode. Some people wake as the heart beats forcefully or irregularly. Atrial fibrillation can produce an uneven rhythm. Anaemia, thyroid overactivity, and heart failure can also be relevant, especially when fatigue, breathlessness, swelling, or reduced exercise tolerance are present.
Steps after an episode
Sit up gradually, settle your breathing, and note the time and your symptoms. Avoid taking extra medicine unless a clinician has instructed you to do so. Review evening caffeine, alcohol, heavy meals, and fluid intake. Keep a brief sleep-and-symptom diary, including snoring or waking short of breath. Arrange an appointment when episodes recur, disturb sleep, or occur with daytime symptoms; a clinician may check your pulse, blood tests, and rhythm.
Get urgent help when
Call emergency services for chest pain, severe difficulty breathing, fainting, new confusion, or palpitations with profound weakness. Waking gasping for air or needing to sit upright to breathe also warrants urgent assessment. Do not wait for the next morning if these symptoms are happening now.
