Healing a Black Toenail: What to Know

A black toenail can result from bleeding beneath the nail after impact, repeated pressure from footwear, infection, or pigment arising from the nail unit. A bruised nail may be tender at first and can loosen as the nail grows. The dark area should gradually move toward the tip with growth rather than remain fixed at the base. Not every dark nail is a bruise, so a new mark without a clear injury, a band that widens, or pigment on the surrounding skin deserves examination.

Wear roomy shoes, keep the toe clean and dry, and avoid picking or pulling at a loose nail. Trim only the free edge when it is safe to do so. Do not pierce the nail at home to release pressure. A clinician can decide whether drainage is appropriate soon after an injury; later, nail surgery or another approach may be considered if the nail is damaged or the cause is uncertain. Photographing the nail with the date can help show whether a mark is moving as it grows.

Arrange prompt assessment for severe throbbing pain, a nail that is split or lifted after injury, inability to move the toe, numbness, or a wound with dirt beneath the nail. Seek care for increasing redness, warmth, swelling, pus, or fever. Have any unexplained, enlarging, irregular, or non-moving dark pigment checked, especially when it affects one nail.

Ask whether the appearance fits a bruise, infection, or another nail condition, and whether imaging or follow-up photographs are useful. Discuss footwear changes, pain relief, the need for drainage, and what nail growth change should be expected.
