What may be behind it

After a twist, an ankle sprain can cause swelling, bruising, tenderness, and stiffness. A fracture may cause marked swelling, focal bone tenderness, deformity, or difficulty bearing weight. Ankle osteoarthritis can produce recurring stiffness and a firm feeling around the joint, especially after activity. Gout often causes sudden intense pain with warmth and redness. Infection is less common but needs prompt assessment when swelling is hot and red with fever or a wound.

A hard area may be thickened tissue, swelling around the joint, or the contour of an injured bone. Ankle swelling is a symptom, not a diagnosis; the examination may include movement checks and imaging depending on the history.

What to do now

Protect the ankle from activities that worsen pain. Rest it, raise it when sitting, and use a wrapped cold pack for short periods after a recent injury. Wear supportive footwear if walking is comfortable, but do not force weight through a painful ankle. Avoid tight massage over a hard or very tender area and do not try to straighten a deformed joint. Arrange review if swelling persists, walking remains difficult, or the cause is unclear.

Seek urgent care after significant trauma, for deformity, severe pain, inability to take steps, or rapidly increasing swelling. A hot red ankle with fever, spreading redness, or an open wound needs prompt assessment.
