What should you know about a hard lump beneath the skin?

The useful clues are depth, mobility, duration, tenderness, surface appearance and whether it is changing. A lump close to a hair follicle may become inflamed, while an older injury can leave a firm area of scar tissue. A boil is more likely to be painful, warm and inflamed as infection develops.

A persistent skin lesion can have other explanations, including Basal Cell Carcinoma. That condition can present as a changing or non-healing spot, but the presence of a hard lump does not establish the diagnosis. Most people need an examination rather than a guess based on touch.

Use a ruler only if you can do so without irritating the area, and note whether the lump is growing or becoming less mobile. Take a clear photograph in good light. Do not lance it or apply strong household chemicals. Loose clothing can reduce rubbing when the lump is in a pressure area.

Book a routine dermatology visit when the lump remains unexplained, lasts beyond a short period, or causes repeated discomfort. The clinician may recommend watching it, treating inflammation, removing it, or sampling it. An intralesional steroid injection may be discussed for selected inflammatory lesions, not for every type of lump.

Get prompt care for rapidly spreading redness, marked warmth, increasing pain, drainage, fever or a general feeling of illness. Seek an earlier skin review if the lump bleeds, ulcerates, repeatedly crusts, becomes attached to deeper tissue, or changes in colour or outline.

Ask what layer the lump occupies, what diagnoses fit its appearance and behaviour, whether a scan or biopsy is appropriate, and what treatment would leave the least irritation or scarring. Clarify how soon you should return if it changes.
