What should you know about hard fat lumps under the skin?

A lump beneath the skin may be soft, rubbery, firm, mobile, or fixed, and the feel can change with its depth and surrounding tissue. Some lumps are benign fatty growths or cysts; others are inflamed follicles, boils, or different skin growths. A skin-lump assessment considers how quickly it appeared, whether it moves, and whether the surface has changed. Basal cell carcinoma usually begins as a skin change rather than a hidden fatty lump, but a persistent sore, bleeding spot, or changing growth still warrants examination.

Do not massage, squeeze, or pierce a hard lump. Record where it is and whether its size, tenderness, colour, or mobility changes. A clinician may examine it and decide whether observation or further testing is appropriate. If inflammation is present, an intralesional steroid injection may be discussed for selected lesions; this is different from treating an undiagnosed lump as though it were fat. Treatment depends on the confirmed cause.

Arrange prompt review for a lump that grows quickly, becomes painful, red, hot, or drains. Seek assessment for a fixed lump, a persistent ulcer, repeated bleeding, or a new surface change. Fever or rapidly spreading redness can signal an infection such as a boil and needs timely care. Do not use the absence of pain as reassurance if the lump is new or changing.

Ask what features make the lump more consistent with a cyst, fatty growth, boil, or another skin lesion. Ask whether it needs monitoring, imaging, sampling, or treatment, and which changes should prompt an earlier appointment.
