What may be behind the change

A thick toenail may become yellow, white, brown or crumbly, but colour is not enough to identify its cause. Fungal infection often develops gradually and can affect more than one nail. Tight footwear or repeated impact can make a nail thick, ridged or lifted. Nail psoriasis may produce pits, separation or debris beneath the nail and may occur with plaques elsewhere. An old injury can permanently alter the growth pattern of one nail.

Pain, pressure in shoes, surrounding scaling and changes in other nails are useful details for a clinician. A sample may be recommended before oral antifungal treatment because several conditions look alike.

What you can do now

Choose shoes with enough toe room, keep feet dry, change damp socks and trim nails carefully after they soften. Do not dig under the nail or share clippers. Clean and dry tools between uses. If fungus is confirmed, use the prescribed course consistently; improvement becomes visible only as clearer nail grows forward. Laser nail fungus treatment and oral antifungal treatment are different options with different suitability and limitations.

Book an assessment for persistent thickening, pain, repeated nail lifting, bleeding, discharge or difficulty walking. Seek prompt review for a dark streak or patch that is new or changing, especially when it does not grow out with the nail. If diabetes, poor circulation or reduced sensation affects your feet, do not try to cut a painful thick nail deeply at home.
