Hand swollen after a fall: what to know

A fall can injure skin, ligaments, tendons, or bone. Pain over the wrist or difficulty moving the fingers does not reliably show how serious the injury is. A Distal Radius Fracture is one possible wrist injury after a fall, particularly when the person lands on an outstretched hand, but an examination and sometimes imaging are needed to distinguish it from a sprain.

Look for increasing bruising, tenderness, reduced grip, deformity, or altered sensation. Hand Swelling may also have causes unrelated to the fall, while Rheumatoid Arthritis can present with inflamed hand joints; a clear injury history makes a trauma assessment the immediate priority.

Remove rings and watches before swelling increases. Rest the hand, support it comfortably above heart level, and use a wrapped cold pack for short periods if it feels helpful. Avoid forceful gripping, heat, and attempts to straighten a visibly abnormal finger or wrist. Arrange medical assessment when pain or swelling is more than mild, movement is restricted, or the injury involved significant force.

Seek urgent care for deformity, an open wound over the injury, severe or escalating pain, fingers that are numb, blue, pale, or cold, inability to move the fingers, or rapidly increasing swelling. Prompt assessment is also appropriate when a child, older adult, or person with fragile bones has fallen.

Ask which structures may be injured, whether imaging is needed, how to support the hand, and when gentle movement should begin. Mention the landing position, exact tenderness, and any change in sensation.
