Hand swelling and heart failure: what to know

When the heart cannot move blood effectively, pressure in the circulation can encourage fluid to collect in tissues. Swelling may affect the feet, ankles, legs, or hands and can be more noticeable after being upright. Heart failure-related swelling often occurs with breathlessness, reduced exercise tolerance, tiredness, a cough, or a feeling of heaviness. Hand swelling alone does not establish the cause; medicines, kidney or liver problems, vein issues, injury, and joint inflammation can also contribute.

Contact your heart-care team about new swelling or a clear change from your usual pattern. Take prescribed medicines exactly as directed and do not increase water tablets, restrict fluids, or change salt intake without personalised advice. Note when swelling appears, whether both hands are affected, any breathlessness, and whether shoes or rings feel tighter. If your clinician has given you a daily weight or fluid plan, follow it and report the changes they asked you to report. Remove tight rings while they still come off easily.

Seek emergency care for severe or sudden breathlessness, chest pain, fainting, confusion, blue or grey lips, or inability to speak comfortably because of breathing difficulty. Contact a clinician promptly for rapidly increasing swelling, breathlessness while resting or lying down, a new cough, marked weakness, or a sudden change in your usual fluid pattern. One-sided painful swelling, redness, or warmth also needs assessment because it may have a local cause.
