Hand swelling after hitting something: what to know

A blow can irritate tissues under the skin and cause fluid or blood to collect around the injured area. Tenderness, bruising and stiffness can follow. Pain over one bone, difficulty making a fist, reduced grip, numbness, or a finger that looks rotated can point to a more significant injury. A wrist impact may involve a distal radius fracture; swelling can also be unrelated to the knock if it continues or affects several joints, as in rheumatoid arthritis.

Remove rings promptly before swelling increases. Rest the hand, keep it raised above heart level when practical, and use a wrapped cold pack for short periods. Avoid pressing directly on broken skin and avoid heavy gripping. Gentle finger movement is reasonable if it does not increase pain, but do not force a stiff joint or try to straighten a visibly displaced finger. A clinician can examine the hand and decide whether imaging or support is needed.

Arrange prompt medical assessment if the hand is misshapen, pain is severe, swelling is rapidly increasing, or you cannot move a finger normally. Seek urgent help for pale or blue fingers, cold skin, new numbness, or an open wound near the injury. If symptoms are not steadily improving, assessment can clarify whether the injury is a fracture or another cause of hand swelling.

Ask which structures may be injured, whether an X-ray is appropriate, how to protect the hand while it heals, and which movements are safe. Tell the clinician when the impact occurred, where the pain is greatest, whether rings were removed, and whether sensation or finger colour has changed.
