Hand swelling after cast removal: what to know

Immobilisation reduces normal muscle pumping and leaves joints and soft tissues stiff. When the cast is removed, movement and use can make fluid more noticeable, particularly later in the day. Dry skin, weakness, tenderness, and limited wrist or finger motion may accompany recovery after a fracture such as a distal radius fracture. Swelling should gradually become easier to manage as movement and strength return, although the pace differs between people.

A swollen hand that is increasingly painful, hot, red, numb, or difficult to move may reflect a complication rather than ordinary stiffness. New or persistent swelling can also have an unrelated cause, so describe the whole pattern at follow-up.

Follow the prescribed exercise plan and increase activity in small steps. Raise the hand when resting, move the fingers regularly, and use gentle wrist motion only within the limits given by your clinician. Keep the skin clean and moisturised once permitted, but do not pick at flaky skin. Avoid forceful stretching, heavy lifting, or returning to sport until strength and bone healing have been checked. A therapist can adjust exercises if swelling rises after practice.

Seek urgent assessment for sudden severe pain, rapidly increasing swelling, a hand that becomes blue, pale, cold, or very tight, new numbness, or inability to move the fingers. Contact the fracture team promptly for fever, spreading redness, wound drainage, increasing warmth, or pain that is escalating rather than settling. Do not wait for the next routine appointment if function changes suddenly.

Ask which movements and loads are allowed now, whether a removable support is needed, and how to pace exercises when swelling increases. Ask how healing will be checked and what symptoms should trigger an earlier review.
