Groin rash: what it may be and what to do

A rash in a moist fold may fit intertrigo, while ringworm often has a scaly, advancing edge. Heat rash can show small irritated spots after sweating. Contact dermatitis may follow a new product, fabric, or topical medicine. Atopic dermatitis and psoriasis can involve other areas and tend to recur. Scabies may cause intense itching at night and affect close contacts. A drug rash can appear after starting a medicine and may extend beyond the groin.

Wash gently, dry by patting, and change out of damp clothing. Choose loose, breathable fabric and pause fragranced products, shaving, and any new cream used on the area. Avoid scratching and do not share towels or underwear if infection is possible. Take a clear photograph and list recent products, medicines, exercise, and travel. A clinician can decide whether the rash needs an antifungal, anti-inflammatory treatment, or another approach.

Seek prompt medical care for rapidly spreading redness, severe pain, warmth, swelling, pus, open skin, fever, or feeling unwell. A rash after a new medicine needs timely advice, particularly with facial swelling, breathing trouble, blisters, skin peeling, or mouth or eye involvement. Arrange review when the rash persists, recurs, or does not respond to sensible skin care.

Ask whether the border, scale, and distribution suggest ringworm, intertrigo, dermatitis, psoriasis, scabies, heat rash, or a drug reaction; whether testing is needed; and which treatment should be stopped or started.
