Groin pain radiating to the testicle: what to know

The groin, lower abdomen, hip, and testicle share nearby nerves, so pain may be felt in more than one place. A strained muscle, irritated nerve, hernia, urinary problem, or testicular condition can produce overlapping symptoms. The route of the pain cannot safely identify its source without an examination.

Tell a clinician whether the pain began suddenly, followed lifting or sport, or comes with swelling, a lump, nausea, fever, burning urine, discharge, or blood in urine. A groin bulge may suggest a hernia, while hip clicking or pain with movement may suggest a musculoskeletal source. Testicular symptoms still need direct assessment.

Stop strenuous activity and avoid lifting. Wear comfortable supportive underwear if it reduces movement-related discomfort, but do not delay assessment to test home measures. Do not squeeze, massage, or try to reposition a swollen testicle. A clinician or pharmacist can advise on pain relief that fits your medicines and medical history.

Go to emergency care immediately for sudden or severe testicular pain, rapid swelling, a high-riding or twisted-looking testicle, nausea or vomiting with testicular pain, or pain after an injury. Seek prompt review for fever, urinary symptoms, discharge, a new lump, or pain that persists or returns.

Ask whether you need a testicular and groin examination, urine testing, or imaging. Explain any exercise or lifting trigger and ask when activity can resume. If urgent genital causes are excluded and movement reproduces symptoms, ask whether physiotherapy assessment is appropriate.
