Groin pain when lifting the leg: what it may mean

Short answer
Groin pain when lifting the leg often involves a hip flexor, inner-thigh muscle, or hip joint, but sudden severe symptoms need assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical trigger
- Kicking, sprinting, sit-ups, or a sudden change in load
- Modify
- Avoid high lifts and forceful stretching while pain is active
- Review
- Persistent weakness or altered walking needs assessment
Why this movement can hurt
Lifting the leg asks the hip flexors to work while the pelvis and trunk provide stability. Pain may follow sprinting, kicking, a slip, repeated sit-ups, or another sudden load on the inner thigh. Deep catching or pinching with hip flexion can occur with hip impingement or a labral tear. Stiffness and reduced movement may accompany osteoarthritis. Pain that is felt only with active lifting can differ from pain caused by someone else moving the leg, so this detail helps a clinician. It does not identify the cause by itself.
How to manage the movement
Avoid repeated high lifts, forceful stretching, kicking, and sprinting while pain is active. Use your hands or a step to reduce effort when getting into a car or bed, and keep daily movements small and controlled. Gentle walking is reasonable if it stays comfortable. Cold or heat may feel useful for some people; protect the skin and stop if symptoms increase. If pain persists or strength is reduced, arrange a physiotherapy or medical assessment before returning to sport.
When care is urgent
Get urgent help after a major injury, a sudden pop with marked weakness, inability to bear weight, a visibly swollen leg, numbness, or severe rapidly increasing pain. A fever, red or hot joint, abdominal symptoms, testicular pain, or a groin bulge also needs prompt evaluation. Persistent pain that changes your walking deserves a planned review even without these warning signs.
What to ask
Ask whether the hip flexor, adductor, hip joint, or another structure is involved; which movements are acceptable; and when strengthening can begin. Explain whether lifting the leg is painful only against gravity or also when the leg is moved for you.

Questions people ask
Why does lifting my leg cause groin pain?
The movement loads the hip flexors, inner-thigh muscles, pelvis, and hip joint, so irritation in any of these areas may hurt.
Should I keep lifting the leg to test it?
Do not repeatedly provoke sharp pain. Use comfortable movement and seek guidance if strength or walking is affected.
