Groin pain after ejaculation: what to know

Ejaculation uses coordinated contractions of the pelvic-floor muscles and nearby ducts. Tight or overactive muscles may leave an ache in the groin, perineum, lower abdomen or testicles. Prostate inflammation, a urinary infection or another pelvic condition may cause pain with ejaculation as well, particularly when burning urine, frequency, discharge, blood, fever or pain with sitting is present.

A hip or groin problem can be aggravated by sexual positioning or muscle bracing rather than by ejaculation itself. Pain that is sudden and severe, focused in one testicle, or accompanied by swelling needs urgent attention because location and timing cannot safely distinguish the cause.

Pause sexual activity if it reliably triggers pain and record when symptoms begin, how long they last and whether urinary or scrotal changes occur. Drink normally, avoid strenuous lifting during a flare, and do not press or massage a swollen testicle. Arrange review with a primary-care clinician or urologist for recurring pain. The examination may include the abdomen, groin, genitals and pelvic floor, with urine testing or other tests when indicated.

Seek urgent care for sudden intense testicular pain, rapid swelling, a high-riding or rotated testicle, fever with severe pelvic pain, vomiting, inability to pass urine, blood in urine, or a painful groin lump. Prompt assessment is also needed if pain is worsening or you feel faint or seriously unwell.

Ask whether pelvic-floor tension, infection, prostate inflammation, a hip problem or another cause best fits the pattern; whether urine testing is needed; whether pelvic-floor physiotherapy is appropriate; and which symptoms require immediate review.
