What should you know about gout back pain?

Gout most often inflames peripheral joints, while back pain commonly comes from muscles, discs, nerves, joints of the spine, or other conditions. Spinal gout is unusual and cannot be diagnosed from pain location alone. Lumbar spondylosis can produce aching or stiffness in the lower back and may be considered during assessment.

Tell a clinician whether the pain started after lifting or injury, travels into a leg, changes with movement, or comes with fever or urinary symptoms. Your medicines and medical history also matter.

Keep gently active within your comfort, avoid heavy lifting while the pain is acute, and use a supported position for rest. Take pain medicine only if it is suitable for your health and other medicines. Do not assume that changing gout treatment will settle back pain; the cause needs to be identified.

Seek urgent care for new loss of bladder or bowel control, numbness around the groin, worsening leg weakness, fever with severe back pain, or pain after significant injury. Prompt assessment is also needed for unexplained weight loss, a history of cancer, or pain that is steadily worsening or disturbing sleep.

Ask what structures may be causing the pain, whether your nerve function is normal, whether imaging is appropriate, and how to move safely. Ask separately whether your gout needs review and which symptoms would change the urgency.
