What should you know about giant dermatofibroma?

Dermatofibromas are firm growths in the skin, often with a smooth or slightly uneven surface. A larger-than-expected nodule may be called giant, but size alone does not establish the diagnosis. Colour, depth, borders, texture, growth pattern, and symptoms all matter.

A clinician may use a dermatoscope and may recommend a biopsy or complete removal so the tissue can be examined. This is particularly useful when the nodule is changing, atypical, or difficult to distinguish from another growth.

Arrange a dermatology examination for a new or enlarging firm lump. Take a clear photograph beside a ruler and note changes in colour, surface, tenderness, bleeding, or crusting. Avoid picking, squeezing, or repeatedly rubbing it, which can cause irritation and make its surface harder to assess.

If removal is recommended because of uncertainty, symptoms, or repeated friction, surgical excision removes the nodule with a margin of surrounding skin and allows laboratory examination. The clinician will discuss wound care and the possible scar before proceeding.

Seek prompt dermatology review if the lump grows quickly, becomes painful, bleeds without obvious injury, develops an ulcer or persistent crust, or changes in colour. Arrange earlier assessment if it is on a site repeatedly caught by clothing or jewellery. Rapid change does not prove a serious diagnosis, but it should not be ignored.

Ask whether the diagnosis is clear from examination, whether a biopsy is appropriate, what removal would involve, where the scar may sit, and how the tissue result will be communicated. Ask how to monitor the area if observation is chosen.
