Gestational hypertension with severe features is a pregnancy-related blood pressure disorder that needs prompt medical assessment, close monitoring, and a personalised plan for treatment and birth timing. It may be diagnosed when blood pressure is markedly high or when tests or symptoms suggest that the condition is affecting an organ, even if you do not feel unwell.

Gestational hypertension begins during pregnancy in someone who did not have known high blood pressure beforehand and does not have the defining protein findings of pre-eclampsia at diagnosis. The term severe features signals concern about the intensity of the blood pressure or possible effects on the body. Clinicians may check for severe headache, vision changes, pain beneath the ribs, shortness of breath, low platelets, changes in kidney or liver tests, or fluid in the lungs.

The placenta and baby are also assessed. Blood pressure disorders can reduce placental blood flow, which may affect growth or the baby's wellbeing. A diagnosis does not automatically mean that birth must happen immediately; the decision depends on pregnancy stage, test results, symptoms, response to treatment, and the safety of continuing the pregnancy.

Follow the maternity team's instructions about hospital assessment, blood-pressure checks, blood and urine tests, fetal monitoring, and medication. Treatment may include medicine to lower blood pressure and medicine to reduce the risk of seizures when severe features are present. Your team may also discuss corticosteroids if an early birth could be needed, depending on the stage of pregnancy.

Take prescribed medicine exactly as directed and keep every monitoring appointment. Do not change a dose, stop treatment, take someone else's blood-pressure medicine, or rely on home readings alone. If you are checking at home, record the readings with the time and any symptoms, using the technique your clinician showed you.

Seek urgent maternity care for a severe or persistent headache, blurred vision or flashing lights, severe pain under the ribs or in the upper abdomen, sudden breathing difficulty, chest pain, fainting, confusion, a seizure, or sudden marked swelling of the face or hands. Contact the maternity unit promptly if the baby is moving less than usual, you have vaginal bleeding, or you feel suddenly much worse.

These symptoms can indicate complications that require immediate examination. Do not wait for the next appointment or assume that a normal home reading rules out a problem. If a seizure, severe breathing problem, or collapse occurs, call emergency services and follow their instructions while help is coming.

Ask which findings make your case severe, how often your blood pressure and laboratory tests will be checked, and which symptoms should take you directly to hospital. Ask how the baby will be monitored, what would make delivery safer, which medicines are planned, and what follow-up you will need after birth.
