Gestational Hypertension Nursing Diagnosis

A nursing diagnosis for gestational hypertension describes the patient’s responses and risks, guiding observation, education, escalation, and support rather than replacing the medical diagnosis. Nursing care is individualised to blood-pressure findings, symptoms, test results, pregnancy stage, and fetal wellbeing.

A nurse may monitor blood pressure trends, headache, vision changes, upper-abdominal discomfort, breathing, swelling, urine output, and emotional wellbeing. They may check whether the patient understands medicines and follow-up instructions, while coordinating urine tests, blood work, fetal monitoring, and clinician review. Nursing priorities can include risk of worsening blood pressure, reduced organ perfusion, anxiety, or gaps in knowledge. These labels describe care needs and can change as the assessment changes.

Report symptoms promptly and answer questions about medicines, health history, and fetal movement as accurately as you can. Learn how to sit and use a home blood-pressure monitor if one is recommended, and keep a written record. Follow fluid, activity, appointment, and medicine instructions from your team. Ask the nurse to explain unfamiliar terms, the reason for each test, and what action to take if a result or symptom changes.

Seek urgent maternity assessment for a severe or continuing headache, visual changes, severe upper-abdominal pain, sudden facial or hand swelling, shortness of breath, chest pain, confusion, fainting, bleeding, or reduced fetal movement. Nurses escalate these findings because they can signal preeclampsia or another urgent complication. Do not wait for a routine nursing review when you feel suddenly or seriously unwell.

Ask what your nursing care plan is monitoring, which findings require immediate escalation, and how results will be shared with the obstetric team. Clarify your medicine schedule, home-reading technique, fetal-monitoring plan, discharge advice, and who to contact overnight or after birth.
