What should you know about genital area itching and rash?

The genital skin is sensitive to moisture, friction, soaps, wipes, sanitary products, lubricants, condoms, and laundry products. Contact dermatitis may cause burning or an itchy rash after exposure, while atopic dermatitis or asteatotic eczema can produce dryness and cracking. A skin yeast infection may cause intense itch and soreness in warm, moist folds. Psoriasis or lichen planus can affect genital skin with less obvious scale than on other body sites. Scabies is another possibility, particularly when itching is severe at night or other household members itch. Symptoms alone cannot reliably identify the cause, and sexually transmitted infections may also need consideration based on exposure and associated symptoms.

Wash gently with lukewarm water or a fragrance-free cleanser, rinse well, and pat dry. Wear loose, breathable underwear and change out of damp clothing. Stop new scented products and avoid douching, scrubbing, or applying steroid, antifungal, or antibiotic creams without advice, because the wrong product can worsen irritation or mask the diagnosis. Do not share towels, and avoid sexual contact if sores, discharge, or pain make infection possible until you have been assessed. A clinician may inspect the skin and arrange swabs or other tests when needed.

Arrange medical review if the rash lasts, returns, spreads, or affects sleep. Seek prompt care for severe pain, blisters, open sores, swelling, pus, fever, painful urination, unusual discharge, bleeding, or a new lump. Urgent assessment is needed for rapidly increasing swelling, difficulty passing urine, or severe genital pain. Pregnancy, diabetes, immune suppression, and symptoms after a new sexual exposure lower the threshold for timely review.

Tell the clinician when the itch began, where it started, products or medicines used, and whether there is discharge, pain, urinary change, or a new sexual exposure. Ask whether testing is needed, which products are safe, and how to prevent recurrence.
