On an image, successive layers of new bone can resemble the rings of an onion. This is a description of an imaging pattern, not a diagnosis by itself. In the jaw, the pattern may accompany chronic irritation from a tooth or nearby infection. Similar layered appearances can have other explanations, so the image must be matched with the examination and history.

The clinician may look for tenderness, expansion, dental disease, drainage, restricted movement, or signs of active infection. The apparent layers do not by themselves reveal whether treatment should be dental, medical, surgical, or observational.
