What should you know about gallstones and sludge?

Sludge is a mixture of bile pigments, cholesterol and tiny particles seen in the gallbladder. It is softer than a formed stone and can sometimes clear when bile flow improves. In other situations it persists, blocks a duct, or provides material from which Gallstones develop.

Some people have no symptoms and discover sludge during an ultrasound for another reason. Others develop steady upper abdominal pain, indigestion, nausea or vomiting, particularly when a duct is irritated or blocked. These symptoms can have other causes, so the scan must be interpreted alongside your examination.

Arrange a GP Consultation if you have repeated upper abdominal discomfort or a scan has reported sludge. Take the report with you and mention pregnancy, rapid changes in weight, recent illness, fasting, medicines and previous attacks. Your clinician may recommend observation, repeat imaging or specialist review depending on symptoms.

Eat regular, balanced meals and avoid foods that clearly trigger pain rather than cutting out whole food groups without advice. Avoid rapid weight-loss plans. Treatment is guided by symptoms and complications; medicine or surgery is not automatically required for an incidental finding.

Get urgent medical care for severe or persistent abdominal pain, fever, yellow skin or eyes, dark urine, pale stools, repeated vomiting, or marked weakness. These signs may reflect bile-duct blockage, gallbladder inflammation or Pancreatitis and need prompt assessment.

Ask what the scan actually showed, whether the sludge is causing your symptoms, whether stones or duct blockage are present, when repeat imaging is useful, and which symptoms mean you should seek urgent care.
