What should you know about frontal scarring alopecia?

Scarring alopecia damages the follicle and replaces it with scar tissue, leaving skin that may look smooth or shiny and lack visible follicle openings. Frontal fibrosing alopecia is one condition that can affect the front hairline and may also involve the eyebrows. It can begin subtly with recession, itching, pain, redness, or small bumps. A scalp examination, dermoscopy, and sometimes a biopsy help distinguish scarring inflammation from non-scarring hair loss or isolated bald patches. The diagnosis matters because the treatment goal is first to quiet activity and preserve follicles that remain.

Arrange a dermatology appointment if the hairline is receding with scalp symptoms, eyebrow thinning, or a change in skin texture. Avoid tight hairstyles, scratching, and harsh chemical treatments while the scalp is being assessed. Treatment is individualised and may include anti-inflammatory medicines, topical care, or injections of steroid medicine into selected skin areas. Keep follow-up appointments even if shedding appears slower, because activity can be difficult to judge from appearance alone. Hair restoration is a separate discussion and is usually considered only after inflammation has been stable.

Seek prompt review for rapidly advancing hairline loss, increasing pain or itch, redness, scaling, pustules, crusting, or new eyebrow loss.
