Foot swelling after knee surgery: what to know

Short answer
Foot swelling after knee surgery can result from normal fluid movement and reduced activity, but sudden or worsening swelling needs medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Swelling can move down toward the foot when the leg is dependent.
- Helpful record
- Note changes in swelling, pain, warmth, colour, wound appearance, and breathing.
- Urgent warning
- Breathing symptoms or sudden one-sided leg changes need immediate medical help.
The short answer
Swelling may collect below the operated knee because gravity draws fluid down the leg when you sit or stand. Dressings, a brace, and spending more time in bed can also slow circulation and limit ankle movement.
What the swelling may mean
Mild swelling that gradually settles with rest, elevation, and the exercises approved by your surgical team is often part of recovery. It can be more noticeable at the foot and ankle than at the knee. Bruising, tightness, and a heavy feeling may occur alongside it.
One-sided swelling with calf pain, warmth, redness, or tenderness can signal a blood clot and should not be assessed at home. Swelling with increasing wound redness, drainage, fever, or worsening pain may point to infection or another postoperative problem. A swollen knee can have many causes; it does not by itself identify a specific condition such as a Baker's Cyst or Gout.
What you can do
Follow the discharge instructions for walking, exercises, compression, and medication. When resting, raise the leg as directed, keeping the foot supported rather than letting it hang down. Gentle ankle movements may help maintain mobility if your surgeon or physiotherapist has approved them.
Do not massage a newly swollen calf or change your bandage, brace, or medication plan without advice. Check both legs for a clear change in size, and note whether the swelling improves overnight or becomes progressively tighter during the day. Contact the surgical team if the pattern is new, persistent, or different from what you were told to expect.
When to seek care
Seek emergency help for sudden shortness of breath, chest pain, coughing blood, fainting, or severe breathing difficulty. These symptoms can indicate a clot that has travelled to the lungs.
Get urgent medical advice for rapidly increasing swelling, new calf pain, marked warmth or redness, a cold or pale foot, numbness, severe pain, or an inability to move the toes. Also contact your surgical team promptly for wound drainage, spreading redness, fever, or swelling that is not easing as recovery progresses.
Questions for your doctor
Ask: Is this amount and location of swelling expected for my operation? Which elevation position and ankle exercises are safe? Should I use compression, and what warning signs mean I should stop? If the cause is uncertain, ask whether you need an examination or imaging before starting treatment.

Questions people ask
How should I position my leg when my foot is swollen after knee surgery?
Use the elevation position recommended by your surgical team, with the leg supported and the foot higher when appropriate. Avoid positions that put pressure on the wound or make the knee brace unsafe.
Should I worry if the foot is more swollen at the end of the day?
Dependent positioning and activity can make swelling more noticeable later in the day. Tell your surgical team if it is worsening, painful, warm, red, one-sided, or not improving with the approved recovery plan.
Can I massage the swollen foot or calf?
Do not massage new or unexplained calf swelling until a clinician has advised you, because a blood clot must be considered when swelling is one-sided or accompanied by calf pain or warmth.
