What should you know about a foot cast for a fracture?

A cast holds the foot and nearby joints in a controlled position. It may be made from plaster of Paris or fibreglass, depending on the injury, swelling, and the clinician’s plan. Some fractures need a cast after the bones have been aligned with closed reduction; others need fixation surgery before or instead of casting. The cast may cover the foot and extend around the ankle or lower leg.

Keep the cast dry and do not push objects inside it to scratch. Elevating the foot can help limit swelling, especially during the early part of recovery. Only put weight through the cast when your treating team has specifically allowed it.

Check the skin at the cast edges and look at the toes regularly. Use crutches or another aid exactly as shown, keep walkways clear, and follow the advised exercises for joints that are not enclosed. Do not trim, loosen, or remove the cast yourself. Attend planned checks so alignment, swelling, and healing can be assessed. After removal, the foot may feel weak or stiff; physiotherapy can help restore movement, strength, and walking confidence.

Contact the treating service promptly if pain or tightness is increasing, the toes become numb, pale, blue, unusually cold, or difficult to move, or swelling does not settle with elevation. Seek urgent assessment for a wet, cracked, loose, or badly damaged cast, new drainage or bad odour, fever, or pain that is not controlled by the plan you were given.

Ask which movements and amount of weight are safe, how to protect the cast while bathing, when it should be checked or removed, and which symptoms mean you should return sooner. Clarify how you will transition from the cast to a shoe or boot and whether rehabilitation is needed.
