Food poisoning without vomiting: what to know

Short answer
Food poisoning can occur without vomiting; diarrhoea, abdominal cramps, nausea or fever may still occur, so hydration and warning signs matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible symptoms
- Diarrhoea, cramps, nausea, fever or tiredness
- First step
- Sip fluids regularly and consider oral rehydration
- Urgent warning
- Blood in stool, severe pain, confusion or very little urine
What no vomiting means
Vomiting is only one possible response to contaminated food or drink. Irritation or infection in the gut can instead lead to loose stools, cramping, nausea, fever, tiredness or reduced appetite. Symptoms may begin after a delay, and the food that caused them may not be the last meal you ate. Not vomiting does not rule out food poisoning, but symptoms can also have other causes. A clinician may consider your symptoms, recent meals, travel, medicines and contact with anyone else who is unwell.
What to do at home
Take frequent small sips of water or an oral rehydration drink, especially after each loose stool. Oral rehydration therapy replaces water and salts more effectively than relying on very sugary drinks. Rest, eat small plain meals when you feel able, and avoid alcohol and foods that clearly worsen symptoms. Wash hands with soap and water and avoid preparing food for other people while you have diarrhoea. Keep track of how often you pass urine and whether it is becoming unusually dark; these changes can signal dehydration. If drinking becomes difficult, ask about oral rehydration therapy or IV fluid therapy.
When to seek medical care
Seek urgent medical help for confusion, fainting, severe weakness, inability to keep fluids down, very little urine, blood in the stool, severe or worsening abdominal pain, or a high fever with marked illness. Contact a clinician sooner if you are pregnant, older, immunocompromised, or caring for a young child, because dehydration can develop more quickly. Arrange medical advice if symptoms are not settling or are interfering with drinking and normal activity.
Questions for your doctor
Could another illness explain my diarrhoea without vomiting? Do I need a stool test, a rehydration plan, or advice about returning to work or preparing food? Which warning signs should prompt urgent review for me?
