What should you know about florid LCIS pathology outlines?

The pathologist examines the arrangement of cells within the breast lobules and may comment on how extensively the lobules are expanded. The outline can mention necrosis, calcification, pleomorphic features, margins in an excision specimen, and whether the cells show loss of normal cell-adhesion staining that supports a lobular pattern. Terminology varies between reports, so the exact wording matters.

Florid LCIS remains non-invasive, but its extent can make imaging and pathology correlation especially important. A core biopsy may not show the whole abnormal area. The radiologist, surgeon, and pathologist therefore compare the report with the imaging target and the procedure used to obtain the tissue.

Ask the breast team to explain every qualifier in the report and whether the result is concordant with imaging. They may discuss additional sampling or excision, specialist review, closer surveillance, and risk-reducing endocrine therapy. Bring the pathology report and imaging summary to appointments. A second pathology opinion can be useful when the wording is unusual or the management decision is uncertain.

Seek clarification if a pathology result has arrived without a follow-up plan. After biopsy or excision, report fever, increasing redness, drainage, worsening swelling, or bleeding. Contact the breast team for a new lump, nipple change, skin dimpling, or unusual discharge. Go to urgent care for uncontrolled bleeding, fainting, severe breathlessness, or severe rapidly worsening pain.

Ask what makes the finding florid, whether necrosis or pleomorphic features are present, whether the margins are clear when relevant, whether imaging and pathology match, and whether further sampling or surveillance is recommended.
