Fixed hives: what to know

Fixed hives are itchy raised welts that can recur in a familiar area, but persistent marks need medical assessment. Individual welts often change shape or location rather than staying identical. Hives may be triggered by infection, heat, pressure, medicines, foods, or may have no obvious single trigger.

Hives are caused by temporary release of inflammatory chemicals in the skin. The rash may be pale in the centre, red around the edge, itchy, or associated with deeper swelling. Swelling around the lips, eyelids, hands, or feet can occur with hives and is called angioedema. A lesion that stays in one exact spot, bruises, hurts more than it itches, or leaves a mark should be examined because it may not be ordinary urticaria.

Note when the welts appear and whether heat, exercise, pressure, a new product, illness, or a medicine came first. Keep the skin cool, wear loose clothing, and avoid scratching. A pharmacist or clinician can advise whether an oral antihistamine is suitable and how to use it safely. Do not start oral corticosteroids without medical direction; they are reserved for selected situations and require attention to personal health factors.

Call emergency services for swelling of the tongue or throat, wheezing, trouble breathing, trouble swallowing, confusion, or faintness. Seek prompt assessment when hives are accompanied by vomiting, severe abdominal pain, fever, purple marks, blistering, or rapidly increasing swelling. Arrange a routine review if episodes keep returning, disturb sleep, or do not behave like brief migrating welts.
