First-Time Spotting After Your Period

Spotting means light vaginal bleeding outside your expected menstrual flow. It may happen when hormone levels shift, after sex, or when contraception changes. Cervical dysplasia, chlamydia, polyps, and fibroids can also cause bleeding between periods. Cervical cancer and womb cancer are less common explanations, but abnormal bleeding is one presentation of these conditions and should not be ignored. A symptom alone cannot identify the cause.

Record the date, colour, amount, triggers, pain, discharge, and any change in contraception. Take a pregnancy test if pregnancy is possible, and arrange a clinician appointment if the bleeding returns, follows sex, or comes with pelvic pain or unusual discharge. A clinician may ask about your cycle and medicines, examine the cervix, test for infection, or arrange imaging based on your history.

Seek urgent care for heavy bleeding, severe or one-sided pelvic pain, faintness, marked dizziness, fever, or a positive pregnancy test with bleeding. Prompt review is also appropriate after menopause, with bleeding that keeps recurring, or when bleeding is accompanied by foul-smelling discharge. Emergency symptoms reflect the need to assess blood loss, infection, or pregnancy-related problems rather than the likelihood of a particular diagnosis.

Ask whether you need a pregnancy test, infection screening, cervical examination, or ultrasound. Tell the clinician when your last period started, whether sex triggers the spotting, and any medicines or contraception you use.
