Fibromyalgia and rheumatoid arthritis: what to know

Rheumatoid arthritis is an inflammatory autoimmune condition that can cause swollen, tender joints, often in the hands and feet. Fibromyalgia is associated with widespread pain, fatigue, and altered pain processing; it does not cause the same joint inflammation. A person may have both, so persistent symptoms should not be attributed to one diagnosis without review.

Morning joint stiffness, hand swelling, finger stiffness, and joint swelling are useful details to record. Their pattern, examination findings, and tests help a clinician distinguish active inflammation from widespread sensitivity or another cause.

Arrange a clinical review if pain, stiffness, or fatigue is affecting sleep, work, or movement. Bring a list of medicines and note which joints swell, when stiffness is worst, and whether symptoms change with activity. Treatment may combine disease-directed care for rheumatoid arthritis with pacing, graded movement, sleep support, and pain-management strategies for fibromyalgia. Anti-inflammatory medication may help inflammatory pain but is not suitable for everyone and should be used only as advised.

Seek urgent medical advice for a suddenly hot, very swollen joint, fever with joint swelling, severe weakness, new chest pain, or difficulty breathing. Contact your clinician promptly when swelling persists, several joints become difficult to move, or symptoms rapidly worsen, because untreated inflammation can damage joints.
