When a fibroadenoma is painful: what to know

Short answer
A fibroadenoma can be tender or painful, but new, persistent, or worsening breast pain with a lump deserves clinical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pain pattern
- May be tender, pressure-related, or linked with hormonal change
- Assessment
- Examination and sometimes ultrasound or biopsy
- Treatment discussion
- Symptom care or excision may be considered
Why it may hurt
Fibroadenomas are solid lumps of glandular and connective tissue and are often painless, but tenderness can occur when the lump is under pressure or when surrounding breast tissue changes. Hormonal shifts, pregnancy, breastfeeding, or chest-wall strain can also cause pain near a lump. Pain alone cannot identify its cause.
A clinician can check whether the pain is centred on the lump, follows a cycle, or comes from the skin, ribs, or muscle. Ultrasound may help describe the lump. A biopsy may be advised if the examination and imaging do not provide a clear explanation.
Ways to prepare for review
Keep a brief record of when the pain occurs, how long it lasts, whether movement or touch affects it, and whether it changes with your menstrual cycle or hormonal treatment. Wear a supportive, comfortable bra and avoid repeatedly pressing the area. Use only pain relief that is suitable for you, especially if pregnant or breastfeeding.
If the lump is confirmed as a fibroadenoma but pain continues, your clinician can discuss monitoring, symptom measures, or breast lump excision. Removal is not automatically needed; the decision depends on the diagnosis, symptoms, and your preferences.
When pain needs prompt care
Seek prompt assessment for severe or rapidly worsening pain, redness and warmth with fever, a rapidly enlarging lump, skin dimpling, nipple inversion, spontaneous or bloody discharge, or a new armpit lump. These changes can signal infection or another condition requiring timely examination.
Questions for your appointment
Ask whether the pain appears to arise from the fibroadenoma or nearby tissue, whether ultrasound is appropriate, and when biopsy would be useful. Ask which pain measures are safe for you and what change should lead to earlier review.

Questions people ask
Can a fibroadenoma cause breast pain?
Yes. A fibroadenoma can be tender, although pain may also come from surrounding breast, chest-wall, or hormonal changes.
Should a painful lump be removed?
Not always. Removal depends on confirmed diagnosis, ongoing symptoms, growth, and the discussion with your clinician.
