Fever with a skin rash in a child

Viral illnesses can cause fever and widespread spots, while heat rash, contact irritation, eczema, or a medicine reaction may affect the skin for different reasons. Ringworm often forms a scaly ring, scabies may itch strongly at night, and intertrigo tends to affect warm skin folds. Note whether the rash blanches when gently pressed, whether it is painful or itchy, and whether it started after a new medicine. A photograph and timeline can help the clinician, but a child’s appearance and responsiveness matter more than the label of the rash.

Offer frequent fluids or usual feeds and keep clothing light. Let the child rest, avoid overheating, and use gentle products rather than perfumed creams. Do not give aspirin to a child, and check the label and dose instructions before using any fever medicine. Avoid antibiotics, leftover prescriptions, or anti-itch treatments unless a clinician or pharmacist has advised them for this child. Keep the child away from vulnerable contacts if a contagious illness is possible, and wash hands regularly.

Seek emergency help if the child has difficulty breathing, blue or grey lips, swelling, a seizure, severe drowsiness, confusion, a stiff neck, or is difficult to wake. Get urgent assessment for purple or bruise-like spots, a rapidly spreading rash, skin peeling, painful blisters, severe dehydration, or a child who looks very unwell. Babies and children with weakened immunity need a lower threshold for prompt medical advice.

Ask whether the child needs an examination, how to monitor fluids and fever, whether school or nursery attendance should pause, and which symptoms should trigger immediate reassessment.
