Fever and tongue changes: what to know

A dry or coated tongue commonly appears when fever, reduced drinking or vomiting lowers moisture in the mouth. Viral illnesses may cause a sore throat and sensitive tongue. A bright red, swollen tongue with a widespread rash can occur in some infections and should be assessed alongside the person’s other symptoms.

White patches that do not wipe away, ulcers, marked swelling or pain may have causes unrelated to fever, including irritation, medication effects or oral infection. The timing, colour and texture are useful clues.

Take frequent sips of water, keep the mouth clean with gentle brushing, and avoid very hot, spicy or acidic foods while the tongue is sore. Do not scrape aggressively or use another person’s mouth medicine. Check for urination, alertness and the ability to swallow.

Get emergency help for tongue or throat swelling, drooling with difficulty swallowing, noisy breathing, facial swelling, confusion or collapse. Arrange prompt care if fever persists, mouth pain prevents drinking, patches spread, the tongue becomes very painful, or a child appears unusually sleepy or unwell.

Ask whether the tongue appearance suggests dehydration, a mouth infection or another illness, and which drinks, rinses or medicines are appropriate.
