Fever with a rash: what to know

Fever with a rash has several possible causes, so describe the rash and check for warning signs rather than guessing. Record the temperature, the rash's first location, how quickly it is spreading, and any pain, itching or breathing change.

Viral infections can cause fever, tiredness and widespread spots; chickenpox often causes itchy spots that change over time. A localised, warm and tender red area can be cellulitis. Cough and congestion may accompany a common cold, acute bronchitis or COVID-19. In a baby or child, a rash with a fever deserves closer observation because hydration and behaviour can change quickly. Appearance alone cannot confirm a diagnosis.

Encourage small, frequent drinks and continue usual feeds when possible. Dress lightly, allow rest, and avoid new creams or medicines unless recommended. Do not pop blisters. Wash hands, avoid sharing cups and towels, and ask a clinician how long to limit contact if an infection may spread. Take a clear photograph and note changes to show the healthcare professional.

Seek emergency help for breathing difficulty, collapse, confusion, a seizure, a stiff neck, severe pain, or a rash that stays visible when pressed. Get urgent advice when drinking falls, urine becomes much less frequent, the mouth is dry, or the person is hard to wake. Arrange medical review for fever that continues or returns, a rapidly changing rash, or a very young infant with fever.
